Revascularization for unprotected left main stem coronary artery stenosis stenting or surgery.

چکیده
For coronary artery disease with unprotected left main stem (LMS) stenosis, coronary artery bypass grafting (CABG) is traditionally regarded as the "standard of care" because of its well-documented and durable survival advantage. There is now an increasing trend to use drug-eluting stents for LMS stenosis rather than CABG despite very little high-quality data to inform clinical practice. We herein: 1) evaluate the current evidence in support of the use of percutaneous revascularization for unprotected LMS; 2) assess the underlying justification for randomized controlled trials of stenting versus surgery for unprotected LMS; and 3) examine the optimum approach to informed consent. We conclude that CABG should indeed remain the preferred revascularization treatment in good surgical candidates with unprotected LMS stenosis.

Acute and Long Term Outcomes of Coronary Intervention in Unprotected Left Main Lesions
Introduction : Significant left main coronary artery stenosis jeopardizes the entire myocardium of the left ventricle and has the worst prognosis of any form of coronary artery disease. Coronary-artery bypass grafting (CABG) has been considered as the standard therapeutic approach for such patients. There are limited data on the safety and effectiveness of percutaneous coronary intervention (PC...
متن کاملImpact of the extent of coronary artery disease on outcomes after revascularization for unprotected left main coronary artery stenosis.
OBJECTIVES This study was designed to examine the impact of the extent of coronary disease on long-term outcomes after coronary stenting or coronary artery bypass graft (CABG) surgery for unprotected left main coronary artery (ULMCA) stenosis. BACKGROUND The differential outcome of ULMCA revascularization according to the coronary involvement remains uncertain. METHODS From the MAIN-COMPARE...

BACKGROUND The study served to present the in-hospital and six-month clinical outcome and also the long-term survival data of a consecutive series of patients undergoing stenting for unprotected left main coronary artery (LMCA) disease. METHODS Revascularization with coronary bypass surgery has been generally recommended for treatment of left main coronary stenosis. Improvements in angioplast...
متن کاملEndoluminal stenting for unprotected left main stenosis in Takayasu's arteritis.
We describe 2 patients with Takayasu's arteritis and left main coronary stenosis who were considered poor candidates for surgical revascularization. Elective left main coronary artery balloon angioplasty followed by endoluminal stenting was performed with excellent results. At 3-mo follow-up, one patient had evidence of in-stent restenosis, but the other remained asymptomatic. In selected patie...
متن کاملRandomized comparison of percutaneous coronary intervention with sirolimus-eluting stents versus coronary artery bypass grafting in unprotected left main stem stenosis.
OBJECTIVES The purpose of this randomized study was to compare sirolimus-eluting stenting with coronary artery bypass grafting (CABG) for patients with unprotected left main (ULM) coronary artery disease. BACKGROUND CABG is considered the standard of care for treatment of ULM. Improvements in percutaneous coronary intervention (PCI) with use of drug-eluting stents might lead to similar result...
